Instructions for Swimmer Registration
2011 Spring / Summer Season
Stroke Clinic and Long Course Programs
 

 On-Line Registration (Preferred Method)
(No payment is needed at the time of registration)

Please complete the form and click submit.  Sandy Payne will process your registration. You will receive an email with a document that contains the registration information that you provided along with an invoice.  Bring your signed registration document and your payment to turn into the registration table at the rec. center the first night of practice.
 

You may choose to pay by check or credit card when you complete the on-line registration.  If you choose credit card, you will receive an additional email: a PayPal Money Request. Simple follow the link to the payment processing page on PayPal.com.   Please complete the payment prior to the registration dates.
PayPal will not share your credit card number with us. 
You do not need to have an account with PayPal.

What if I can't register on-line? Simple complete standard method below.

You will need to completely fill out the following forms. (One of each for a family)

* USA Swimming Registration - Required by USA Swimming for all members of MAKOS Long Course.  - $57.00 per swimmer
Membership includes registration card, swimmer insurance benefits and “USA-Swimming Splash”  Magazine subscription.  This fee is non-refundable.
Swimmers who purchased during the winter season are already covered.  Registrations are good till Dec. 31, 2011.

* Family Deposit Account - $50.00 per Long Course family
Will be returned as a credited to your final payment.

Fundraising Fee - Makos does not have any additional required fundraising dollar amount. 
Every long course family (Partial and Full Season) plus Stroke Clinic families that swam for Makos during the 2010-11 short course season are required to work our Mako Hosted Meets. 
Requirement (based on your highest level swimmer):
Bronze, Silver and Gold - 20 Hours per hosted meet.
Stroke Clinic (Existing Mako Families) - 12 Hours per hosted meet.
Stroke Clinic (not part of short course season) - No requirement, but we would love for you to help out also.

If you are unable to meet your requirement, families will be charged at a rate of $5.00 per hour for the hours that they are short of their requirement.  This fee allows us to pay others to work the hours for you.

Miami Rec. Center Membership - NO Membership is required!!

2. Payment Due dates:
Stroke Clinic - 100% first night of clinic

Partial Season Long Course
- 100% first night of practice

Full Season Long Course:
Initial Payment - 50% - First night of practice 
Second Payment - 50% - June 10th

Makos is willing to work with your family if a different payment schedule is needed. 

3.  Mako - Rec. Center Card
Each swimmer is given one card.  This card is your swimmer's pass to get through the rec. center security check point to access the pool.  Do not loose this card.  A $30.00 fee is charged for any lost cards.  These cards will be turned in at the end of the season to the Mako Lock Box.

4.  Parking 
It now required to have a RED Miami University parking pass to park in the two rec. center lots.  Miami DOES ticket!  Please park on the street or in the parking garage.
